You lie awake at 3am, not anxious exactly, but your mind won’t stop running through work, conversations and decisions. Whether it’s a term sheet in London, a trial you’re prepping in Amsterdam, a surgery roster in Miami or a fund close in Singapore, the pattern is the same. This has a name in Chinese medicine, Heart–Kidney disharmony, and it lines up closely with what modern physiology describes as a mistimed cortisol awakening response. If 3am has become your norm, there is a real, physiological reason for it, and a way to work with it.
Understanding 3am insomnia
The usual sleep hygiene advice, dim the lights, cut the caffeine, keep a consistent bedtime, treats 3am waking as a habit problem. In my experience that misses what is actually happening in the body. Cortisol naturally begins a gradual rise between roughly 2am and 4am as part of the normal cortisol awakening response. In someone whose stress-hormone rhythm is already dysregulated, through chronic stress or unstable blood sugar overnight, that rise can come sharp enough to fully wake the person, usually with a racing mind or the “tired but wired” feeling my clients describe so consistently. In Chinese Medicine terms, this window also sits inside the Liver’s own organ-clock time, and connects to the Heart–Kidney communication pattern below. Sleep hygiene still matters, but it rarely resolves a dysregulated hormone rhythm or organ-clock imbalance on its own. If this pattern of exhausted-but-wired sits alongside broader burnout, it is worth reading together with non-restorative sleep and Shen disturbance.
Heart–Kidney disharmony explained
This is one of the clearest patterns I see in clinic, and it is a genuinely classic one in TCM literature. Kidney Yin, the deep, cooling, grounding reserve, fails to rise and anchor Heart Fire and Shen. What results is exactly that “tired but wired” presentation: the body exhausted, the mind racing, waking reliably around 2am to 3am, restless or vivid dreams, sometimes lower-back weakness, night heat or dryness, and a low daytime hum of anxiety or mental fatigue. Treatment works to nourish Kidney Yin, settle Heart Fire and restore the communication between the two, through points that connect the Heart and Kidney channels and, where appropriate, classical formulas built for exactly this pattern.
The role of vagal tone
Your vagus nerve works like a brake for your nervous system, and vagal tone is essentially how well that brake responds. Low vagal tone means less control over the stress response, which shows up at night as exactly this kind of waking. Simple, consistent practices, slow breathing, gentle stimulation around the ear or neck, can measurably improve vagal tone over time. “I’m not even stressed, so why is my mind racing at 3am?”
I hear this constantly, and here is my honest read. The mind is reporting the symptom, not the driver. Mistimed cortisol, a nocturnal blood-sugar dip, residual sympathetic tone, or a Heart–Kidney or Liver Qi pattern can all produce this kind of mental hyperarousal without you feeling stressed during the day at all. The nervous system stays in a low-grade alert state even when your conscious story is “I’m fine.” I see this most in people whose work never really has an off switch, the engineer holding a system in their head at all hours, the partner who cannot stop reviewing a case, the physician still on call in their mind long after the shift ends. High-functioning people adapt so well to chronic load that the body keeps score long after the mind has stopped noticing. If fear or low-grade dread tends to sit underneath your version of this, that connection is worth reading in fear and burnout.
What I actually use to reset it
With a new client dealing with 3am waking, I start with acupuncture, body points or ear points, aimed at settling the Shen and regulating the Heart–Kidney axis or Liver timing, paired with simple vagal breathing practices they can use at home. I lead with this because acupuncture has measurable effects on heart rate variability, cortisol and sleep architecture, and it addresses both the TCM pattern and the modern stress-physiology picture at the same time. The breathing work gives you something immediate to use between sessions and reinforces that parasympathetic tone.

Lifestyle levers, stable blood sugar in the evening, morning light exposure, magnesium where appropriate, a consistent wind-down, get layered in from the first session. But the hands-on regulation comes first, because your nervous system needs a clear “it is safe to rest” signal before habits alone can hold.

The way I think about it, and explain it to clients, is in three honest tiers. What is established: acupuncture points mapping to measurable shifts in HRV, cortisol and sleep architecture, the vagal mechanisms above, the Heart–Kidney pattern itself. What is emerging: quantum biology and bioelectric signalling, a genuine, growing field studying how the body’s own electrical and photonic processes influence state, still early but promising. Executive burnout recovery and sleep
For the founders, engineers, lawyers and clinicians I see most, 3am waking is rarely isolated, it is usually one visible symptom inside a wider pattern of depletion that also shows up as decision fatigue, shorter patience and a baseline that never quite resets on weekends. A founder mid-raise, an engineering lead carrying a system no one else fully understands, a partner before trial, a doctor between shifts, the professions differ but the physiology does not. Recovery here works the same way sleep recovery does: hands-on regulation first, so the nervous system gets a real signal that it is safe to downshift, then the structural changes, boundaries, workload, recovery windows, that keep it from rebuilding.
